Access & Community Based Services Long Term Care Services

  • Information and Assistance (I&A): Connects people to available resources in their community by providing general information, support, and advocacy.

  • Nutrition Services: Group meals served at senior centers, schools and other site; Farmers Market programs, nutrition education and health promotion-related activities.

  • Transportation: Rides to critical destinations such as doctor's office or the grocery store.

  • Caregiver Support: Education and resources that enable caregivers to provide care for an older family member while maintaining their own quality of life.

  • Employment and Volunteer Services: A way to help the individual find meaningful work, or to continue to contribute to their community.
